Advantage and disadvantage Sodium Valproate:
1.Advantage:
- Effective in Treating Seizures: Sodium valproate is primarily used as an anticonvulsant medication to control seizures in epilepsy, including generalized seizures, absence seizures, and partial seizures.
- Mood Stabilization: It is also used to stabilize mood in bipolar disorder, helping to prevent manic episodes and depressive episodes.
- Prophylaxis for Migraines: Sodium valproate can be effective in preventing migraines and reducing their frequency and severity.
- Wide Age Range: It is approved for use in both adults and children, making it suitable for a broad range of patients with epilepsy or bipolar disorder.
- Multiple Dosage Forms: Available in various formulations such as tablets, capsules, and syrup, making it convenient for patients who may have difficulty swallowing pills.
- Fast Onset of Action: It typically starts working relatively quickly after ingestion, providing rapid relief for seizures or mood symptoms.
- Long-term Use: Sodium valproate is considered safe for long-term use in the management of chronic conditions like epilepsy and bipolar disorder.
- Well-Tolerated: It is generally well-tolerated by most patients, with common side effects typically being mild and manageable.
2.Disadvantages:
- Side effects: Epival can cause a range of side effects, including drowsiness, dizziness, nausea, vomiting, diarrhea, weight gain, hair loss, and tremors.
- Liver toxicity: There is a risk of liver damage associated with Epival, particularly in people with pre-existing liver conditions. Regular monitoring of liver function is usually recommended while taking this medication.
- Birth defects: Pregnant women taking Epival are at a higher risk of giving birth to babies with congenital malformations, such as spina bifida, heart defects, and cleft palate. It’s important for women of childbearing age to use effective contraception while taking this medication.
- Interactions: Epival can interact with other medications, potentially reducing their effectiveness or increasing the risk of side effects. It’s important to discuss all medications, including over-the-counter drugs and supplements, with a healthcare provider before starting Epival.
- Cognitive effects: Some people may experience cognitive side effects such as memory problems, difficulty concentrating, or confusion while taking Epival.

Action of Valproic Acid in Epival:
Valproic acid, found in Epival, primarily works by increasing the levels of the neurotransmitter gamma-aminobutyric acid (GABA) in the brain. Elevated GABA levels lead to a calming effect, aiding in seizure control and mood stabilization. It modulates GABAergic neurotransmission by inhibiting the enzyme GABA transaminase and possibly other mechanisms. Valproic acid may also affect voltage-gated sodium channels, reducing neuronal excitability. Furthermore, it inhibits the enzyme histone deacetylase (HDAC), which may contribute to its mood-stabilizing effects in bipolar disorder.
The drug’s efficacy in treating various seizure types and bipolar disorder underscores its multifaceted mechanism of action. Epival’s ability to regulate neurotransmitter balance is crucial for managing neurological and psychiatric conditions. Long-term use requires careful monitoring due to potential side effects like liver toxicity and pancreatitis. Overall, valproic acid’s diverse actions make it a valuable treatment option for epilepsy and mood disorders.
How to use:
The recommended intial dose of epival syrup is 10 to 15mg/kg/day.
This should be increased by 5-10 mg/kg/week until control is achieved or until maximum recommended dosage 60 mg/kg/day is reached .
